Travel Authority
LIST OF REQUIREMENTS FOR TRAVEL AUTHORITY
- Certification from Barangay (LSI* or PIT* Certificate)
- Medical Health Clearance (RHU*)
- Company ID or Certificate of Employment (for APOR*)
- Parent’s Consent/Waiver with ID (for Minor/s)
Means of Transportation / Vehicle Documents
- LTO Permit (for private vehicles transporting passengers)
- Medical Health Clearance of driver (RHU*)
- Driver’s License
- OR* / CR* of Vehicle
- Picture of Vehicle (front, side, back)
Submit the above requirements in Two (2) copies each.
- LSI – Locally Stranded Individual
- PIT – Persons with Indispensible Travel
- RHU – Rural Health Unit
- APOR – Authorized Person/s Outside Residence
- OR – Official Receipt
- CR – Certificate of Registration
Application Process for Travel Authority:
- Secure a Certification for Person with Indispensable Travel (PIT) or Locally Stranded Individual (LSI) from the barangay.
- Secure a Medical Health Clearance from Rural Health Unit (RHU)
- Photocopy all the requirements (PIT/LSI Certificate, Medical Certificate, OR and CR of the vehicle to be used, Driver’s License, Medical Certificate of the driver, picture of the vehicle and Waiver for minors if any) in 2 copies each
- Submit the photocopy of the abovementioned requirements to MPD Office for checking
- Fill out the form to be given by MPDO staff
- Submit the verified requirements to Ragay MPS for the approval and issuance of Travel Authority
-Letter of Acceptance
Application Process for Letter of Acceptance:
- Fill out the Request Form from MPD Office with the necessary information
- Wait for the printed Letter of Acceptance
- Submit to the Punong Barangay the Letter of Acceptance or signature
- Send the letter to the LSI
